Form Offline Page

The Form Offline template is used whenever the form is taken offline. Like the Thankyou Template Type, this should display whatever content was explicitly entered via the Form Builder window for the "FORM OFFLINE PAGE CONTENT" setting.

You can add whatever markup you want, but the standard content of this template type is:
